CT DPH Public Health Internship Program

Student Application Form Fall/Spring 2023-2024
The CT Department of Public Health’s (DPH) Office of Public Health Workforce Development thanks you for your pursuit of a career in public health, and more specifically your interest in an internship placement at a state, local, or non-profit public health agency in our state!

Beginning in 2023, CT DPH revised its system for collecting information from potential student interns and placement sites in order to better match students with host sites that are more appropriate for their mutual interests and physical location.  As such, we ask that you please complete and submit the following form with as much specific information as you can provide so that we can find you the best host site match options available.

Important Notes: 

  • CT DPH will use the information on this form to help match students seeking internships with willing preceptors whose training and experience will likely enhance the student’s learning experience.  However, because the number of available host sites may vary depending on the availability of Preceptors at different times of the year, students may receive a more general placement match than they initially requested.  
  • Internship placements through this program are unpaid, unless otherwise indicated by the host site.  Students should assume that all internship hours will be in-person unless otherwise indicated by the host site.
  • Students are responsible for ensuring that any paperwork or other information required for course credit or otherwise required by their academic program is submitted in a complete and timely fashion.  Students should consider any due dates required by their academic programs and give Preceptors sufficient time to complete any information required from the host site.
